Physicochemical parameters of saliva as indicators of dental caries risk in hypothyroidism

The association between salivary flow disorders and systemic diseases has been demonstrated by many authors; however, there is still insufficient evidence of the influence of hypothyroidism on the reduction of unstimulated salivary flow. Objective: To study the physicochemical parameters of oral fluid as indicators of dental caries risk in hypothyroidism. A total of 63 patients with clinically and biochemically confirmed hypothyroidism and 64 without it were studied. The severity of hypothyroidism was assessed based on data from the endocrinology patient's outpatient records. Patients with elevated TSH and normal free thyroxine (T4) levels were considered latent; those with elevated TSH and decreased free T4 levels were considered manifest. The overwhelming majority of patients with hypothyroidism 37 (58.73%) had high caries intensity versus 20 (31.25%) in the comparison group: average intensity was respectively 19 (30.16%) versus 21 (32.81%) and high intensity was recorded in 37 (58.73%) patients with hypothyroidism versus 23 (35.94%) in the comparison group (χ²= 9.619; P ≤ 0.01). Thus, the severity of the clinical course of hypothyroidism is associated with a decrease in the qualitative and quantitative parameters of saliva: increased dryness in the mouth, increased viscosity, decreased pH and buffering capacity, and a decrease in the amount of saliva produced.
